What to expect on your first visit.
Our intake form is online so it can be filled out at your convenience before you arrive at our clinic. Download and save the fillable PDF form to your computer. Once you have completed the form, you can email it to us at firstname.lastname@example.org or you can print it out and bring it with you to your appointment.
You will be greeted at the front desk and our staff member will ask for your intake form and will make a copy of your driver’s license and insurance card. There may be some additional paperwork to fill out based upon your insurance or other requirements.
You will meet with the doctor one on one where you will be asked questions about your current condition and past health history. This information is important in determining an accurate diagnosis.
After the consultation, a comprehensive assessment and examination are performed next. This includes a functional movement assessment to identify faulty movement patterns, neurological testing, orthopedic tests, joint and muscle testing and gait analysis. This information is used to develop an individualized care plan.
InMotion Chiropractic is “In-Network” with many insurance carriers and most insurance plans are accepted at our clinic including:

Our clinic staff will contact your insurance company to verify your benefits, however, the quoted benefits are not a guarantee of payment. You will be responsible for any non-covered services, deductibles or co-pays. If you have any questions regarding your insurance coverage or financial responsibilities, please do not hesitate to ask.
Naturally, we also welcome patients who do not have insurance or who have an insurance plan that does not cover chiropractic. If you will be paying cash for your care, you will find our prices to be competitive with other chiropractors in the area.
Auto Accidents and Worker’s Comp Injuries
If you are injured in an auto accident or an on-the-job injury, please notify your insurance company or your employer immediately. When you arrive at our clinic for your first visit, please provide us with the name of the insurance company, the claim number, the adjuster’s name and contact information. Please inform our office if an attorney is representing you. Once the claim is settled, or if you suspend or terminate care, any fees for services are due immediately.
